REQUIRES A SEPARATE PERMIT.

1. Applicants name = Your name

2. Business name, if applicable

3. Your current address and phone number

4. Property owner (if different)

5. Area of lot or parcel

6. Location = Street name, 911 address, or distance from nearest driveway.

7. Description of what is going to be done.

8. Area of building or project to be done.

9. Nature of work = self explanatory.

10. Before and after use of structure.

11. Setback from road ( USE CENTER LINE OF ROAD)

12. Building distance from both sides of property lines.

13. Building distance from rear property line.

14. Size of building and number of stories.

16. In the graph, sketch and indicate the building and use arrows to indicate distances from the street, side lines and rear lines. ALL DRAWINGS ARE TO BE DRAWN TO SCALE.

17. Signature(s) are required.

18. AN INSPECTION IS REQUIRED WHEN THE FOOTINGS ARE POURED AND A CERTIFICATE OF OCCUPANCY IS REQUIRED FOR ANY STRUCTURE BEFORE IT IS USED.

19. APPROXIMATE VALUATION of the project to be erected. This is due to new state and federal government regulations. This is not used for taxing purposes.

20. It is your responsibility to check with the State Environmental Office and the State Fire Prevention Division to see if any permit is needed from them. You must check the appropriate boxes, and provide copies of the state permit, if applicable.
